We ask that you fill out the online form by clicking on the link that was emailed to you or you will need to arrive 5 min early to your appointment to fill it out in office.
Cancellations
Please provide us with 24-hour advance notice for cancellation in order to avoid a $25 Cancellation Fee. If you cancel or No-Call/No-Show within less than 24-hour notice 3 times, you will be required to pay the cancellation fee. If you have to be charged the cancellation fee twice, you will no longer be allowed to schedule with us and you will be asked to take your business elsewhere. If you reschedule more than 3 times in a row then you will be canceled and asked to reschedule when you know you can in fact make it.

No video recording or pictures of the private areas will be allowed!
Please make sure that you are on time for your appointment. Anyone more than 10 minutes late, for any reason, will be asked to reschedule.
